Be interesting to see condition of the intake valve seat, as valve was perhaps blocked open by piston debris which allowed remnants to pass "backward" into the port.
Any parts missing from #4 carb?

Maybe, a manufacture defect in the piston?

The scrap metal on the intake side - that may make sense. Once the piston performed its vanishing trick, there's no downward motion of that piston to draw air in - so when the valve opens you have slightly positive crank case pressure that's going to exit the easiest way it can.
I'm curious about what you find when you do tear down fully - I still can't understand how the darned piston just disappeared without the valves being trashed....

That usually takes out the cams & bearing inserts. Having ventilated a few sets of cases in my day, it looks like maybe the pin seized /galled /failed & took out the #4 hole...$.02 CDN., not valid in Quebec.

OMR industries is proud to introduce it's latest go fast goodie. The oil pump relay mod. It's only available for purchase through Zmax enterprises. Look on the website next to the hand bent PVC exhaust systems. Dave also recommends that you poke a hole in your lower case to let out that nasty crank case pressure. Keep those 1015cc pistons moving as efficiently as possible.
Tests have shown that the combination of these mods, plus BS34's with main jets drilled out using a 17/32" bit will create enough horsepower to achieve escape velocity. To the moon Alice!
