throw out keeps popping off when i grab my clutch

hi i have a 1981 KZ550 and i just replaced my clutch plates and cable but now when i pull in my clutch lever the trow out pops off and i have no clutch i have got it now to where i have alot of play in my clutch lever but can get it to disengage but it has popped off while riding as well do i need to replace my throw out or is there somthing i am missing i have spent many hours with the side cover off nad trying to dial it all down and have followed the manual on setting the screw and all of that any suggestions?

What manual are you using? On the 550, as part of the clutch adjustment procedure,
you're supposed to turn the clutch release adjusting screw counterclockwise first until hard to turn,
then clockwise 1/2 turn (or 1/4?).

it a 1/4 turn and thats what i did. but its like its popping off of the ball bering
