Found tucked away in some old documentation. Single, two-sided page. Considering it's on blue paper & was folded multiple times, these scans are the best we can do. Offering up for inclusion in the File Base if deemed worthy.

Windows snipping tool can be used to do a screen capture of the PDF.

That's interesting. It displays OK here on Firefox/WIN10. Both files also render correctly in Acrobat Reader. This is why we tested .pdf scans in the 'About The Site' sub a few days ago. We've had problems with .pdf files displaying correctly on KZR.in the past. Seems hit or miss.
Screenshot of what we see displayed is below. It's a .png from the Firefox screenshot tool. The scroll bars on both images work for us too: Weird. We seldom have issues with .pdf files on other forums.
We'll attach both .pdf files below our signature but not inline. Please indicate if they display OK like that. Might have to right click/save as or otherwise jump hoops. If they still don't display correctly, we'll just scan them as .jpg files.
Wookie, we'll also send both files to you via email.
Apologies for the disruption y'all.
EDIT: Just tested in Micro$oft Edge. The OP renders with the same corruption as in Wookie's response. Also tested in Chrome (not Chromium). Same corruption appears in Chrome. Still renders fine for us in Firefox.

I just clicked one of the images and I was prompted to download the file. I did and I was able to open the file without problem on my not as old iPad.
